• Goal linkage is explicit and matches the current plan
• Intervention is rehabilitative and observable
• Participant response is documented, ideally with a quote
• Progress or barrier is measurable
• Next step includes timing and responsibility
• Language is objective and non-judgmental
• Wording is individualized to this participant
• Content stays inside PRP scope
• Times, duration, location, and signature are complete
